Repeated video image of mask on front, close up of Glover on back. B/w lyric sheet inside (pics of Glover printed back to front, so he seems to be playing left handed). All other versions were substantially the same although AMERICA (21 Records T1-1-9009) put the photos (printed correctly) and lyrics on the record bag.

Contemporary cassette copies of The Mask had an extra track 'UNNATURAL'.
